Is it possible/easy to transfer from a prelim medicine program to another categorical medicine program after your prelim year is done. Hypothetically speaking, say I was applying to an advanced program that requires a prelim year or trans year but I had a change of heart and decided that I wanted to do medicine instead but the prelim program I was at wasn't that strong and I wanted to be a categorical at a top institute. Is it possible to do that? and if so, how difficult would it be relative to the difficulty it would be to get in if I applied straight out of Med school?

If you are having doubts, you should get a prelim at strong program that you would be proud to graduate from should you decide to stay in IM. It is much easier to stay at a program that has gotten to know you; if you make a good impression, they are likely to accomodate you. Versus, trying to find a categorical PGY2 vacancy at a top program while you are in your internship and have very little time. These openings are rare...you would nearly have to put a contract on someone to get thier spot...not that I am suggesting it 😉
