Have a new group of fellows coming in and wanted to give them a broad overview. Searching the web I can't find a graphic that has a flowchart or something like axial pain - facet/ SI; radicular - LESI, etc. if that makes sense. Or even a picture of a torso showing typical distribution of SI vs cluneal etc. I guess I could make one but I'm sure something like this exists somewhere?

I don't know that trying to condense a 1 year fellowship into a flowchart is possible.

I can see that you're trying to help. But things like this is what diminishes our value in theeyess of the general medical community. There's a handful of procedures that we do and narrowing it all down to a chart really defeats the point of fellowship which is to learn more than just sticking a needle in it. I'm saying this from experience because I shadowed a couple of radiology and non pain trained clinics where they had "algorithms" and essentially every patient that came through went through the gamut of essentially everything we offer. If you throw 50 darts at a board eventually one will stick.

I think you could put together a talk that’s in that vein, but I’d stay away from treatments and a flowchart that goes “axial lbp” -> MBB/RFA -> SIJ -> Intracept if modic changes, and so on. Just focus on the typical symptoms and pain patterns of the common and uncommon syndromes. Put up the dermatome and peripheral nerve maps, referral patterns on facet joints, SI joints, hip joints. Red flag complaints. That paper about compression fractures not having pain over the fracture about 1/3 of the time.

Dermatome map
Myotome map
Clearly labeled segments of cervical, thoracic and lumbar spine

A doctor should be able to understand how to correlate radicular and non-radicular pain based on the above. You don’t need to spoon feed an algorithm that a midlevel would need
