It's been a few months since I was on Peds, and I've forgotten a great tool for remembering when the cyanosis of congenital heart defects presents after birth. I know its format is "2 minutes:, 2 weeks:, 2 months:, 2 years:" , but I don't remember which defects go where.

Can anyone help me out? Thanks.

The most important thing to remember is this:

IF the cyanosis develops immediately after birth the most likely diagnosis is transposition of the great vessels

If the cyanosis develops later on the most likely diagnosis is tetralogy of fallot

yes, and if it develops 2-3 days after, and ekg shows left axis dev. it's probably tricuspid atresia, this of course happens 2-3 days after birth as the ductus is closed. give these pt's PGE to keep it open until they baby gets surgery to correct it.

True,

another useful way to remember it is that all the important cyanotic lesions begin with a T

Transposition of the great vessels
Tetralogy of fallot
Tricuspid Atresia
Total Anomalous Venous return.

All the the Important non cyanotic lesions have a D in them

ASD
VSD
PDA
