I've seen two so far. SUNY Stony Brook's secondary essay,

"how will reform effect your decision to enter medicine?"

and in one of OhioU's (DO school) automated mass emails,

"Secondary application materials, if eligible, an initial status information, will be sent to you electronically."

Both these schools should now be required to overlook any applicant's typos. 😛
