UCLA-Olive View and "eval letter"?

UCLA/Olive View provides this letter that is a compilation of what various residents/attendings had to say about you. With 1.5 weeks in each hospital, it is nearly impossible to work more than 2 shifts with one attending. Do people use this eval letter as a LOR? Anyone know anything about this?

Also, I presume 2-3 shifts is too few to hit someone up for a SLOR. Correcto?
