Grading is on a P/F basis not only for individual classes, but also for the academic year itself.

The P/F cut off is based on how your classmates did, since everything is subjected to the bell curve. While I was there, the P/F ranged from a high of about 85 to a low of 32(The higher numbers generally for dental classes, and the lower ones for some of the tougher basic science courses).
 
If a school has strictly P/F grading, how do they rank students, if at all?

They do keep "unofficial" stats which then then use when the dean is writing out evaluations for residencies. Even then though he'll use vague descritpive terms as "top quarter". The "academic promotions committee" or APC which is where they determine if you get promoted from year to year and eventually graduates sees all you numerical grades and how they rate to the pass/fail mark, and while you don't get a numerical class ranking, they can place you, atleast mentally, wher you fall with respect to your classmates, although they'll never actually tell you where that spot is.
