I got it from the source - don't use the recommendation form that is currently on the UCSF website. It is last year's form. The new supplemental application will not be updated until later next month - AND, they're going to be using the PharmCAS electronic letter of recommendation next year. The guy I spoke to said they weren't going to accept any letters at the school - that you had to send them all in through PharmCAS.
