I am so confused at how this would work for a traditional student
Traditionally you apply after your junior year, interviee your senior year..
So when their classes start a trad student would be finishing their last semester..
Okay..
Wow

The majority of UMKC's students are in a 6 year BS/MD. They admit MD only students to fill spots vacated by BS/MD'ers before they reach the MD portion. The application cycle moves quickly so yes if admitted you matriculate in January 2018.
Many schools will allow you to defer enrollment, is it possible to defer the UMKC offer for a semester? If you are a strong candidate for Duke, you should be a strong candidate for other great schools as well. It may seem easy to take the "bird in hand", but in the long run coming from a place like Duke will help you get a top residency, fellowship, etc.
