Uncertain of military discharge status

If they said you hid the fact during meps it could be "other than honorable", Which is a separate category.

It isn't a medical discharge though. ELS is a category entirely unto itself. Generally reserved for people who a commander cannot honestly evaluate and thus discharge honorably (or otherwise).
You can appeal to have your discharge upgraded, but it is likely that you were not in long enough for this to happen. I would contact AMCAS and ask them directly, also contact the Army Review Boards Agency (http://arba.army.pentagon.mil/contact.cfm) and ask them. The 2 pertinent links on that page would be "I want to have my Army discharge upgraded" or possibly "I have a question that was not addressed here." I think if you write them or call them they will definitely be able to give you a 100% clear and accurate answer.
The reason I advocate this instead of just clicking on honorable is because it is technically not an honorable discharge, and I would hate for someone at AMCAS or schools you apply for to hold that against you when your intentions are good.

Separation is completely uncharacterized, which means you shouldn't have to list it at all. It's only real applicability is if in the future you decide to rejoin the military. I wouldn't list it at all if I were you, especially if you can't properly characterize it. Or leave it blank and submit a letter of explanation along with it.

Yeah, I'm pretty sure this is your answer. As I remember if you didn't make it through boot camp you basically never existed.
