I noticed that there were quite a few programs that were unfilled following the match last year. Is this because many of these programs would rather be unfilled than lower their standards to fill their slots? Do most of these postitions get filled in the scramble?

There really weren't that many unfilled spots last year, were there?. Here's what I learned about the match from the other side during my intern year. First off, it takes about 8-10 interviewees for every matched spot on average. So if you have a large program, you will have to interview a whole bunch of people to make that list up. Also not everyone interviewed is ranked. As far as lowering the standards, that comes in to a point because the scramble does have qualified candidates to choose from and then it's a buyer's market. Also, inevitably people will want to switch in from other specialties after a year or even a career and these don't go through the match.
 
programs that don't fill one year usually learn their lesson the second time around. i heard through the grapevine that vcu in richmond just planned on too many of their ms4's staying on and didn't interview/rank enough people. likewise uconn is a really solid program that just met with one of those quirks of getting a lot of people to come interview that were interviewing at the same programs. then they got screwed in the rankings because they were everybody's 4th or 5th choice (ie. those candidates matched higher).

also at my program (which kept one homegrown this year and filled) had about 4-5 ms4's last year who were interested in gas. supposedly they got about 750-800 applications and interviewed about 150 for 12 spots. this year, through interacting with the seniors, there's like 14 or so who are interested in anesthesia, a couple of whom want to stay and others who are pretty honest that they want to move on for various reasons (but will probably still interview with us). so, i think it's going to be a bumper year and you shouldn't expect to see this many spots go unfilled this time around, despite the fact that it was actually less than the '04 match. time will tell.
 
It's also interesting to look at the unfilled spots from year to year. Seems like many places on the 2004 match list filled in 2005 and not vice-versa. Goes to show you that some people look at those lists pretty closely when applying and make an attempt to apply to the unfilled programs as 'backups'. Can't say that I did it much differently 2 years ago myself...
