General Unproductive gap year after undergrad - problem?

If someone doesn’t do anything related to medicine for 1 out of 2 gap years, does that look bad?

They spent the time on family issues (including supporting the family business). They kept up with clinical and non-clinical volunteering. However, after this year, they were productive, like working in a clinic full time. Just want to know how big of a red flag it is to not be "officially" employed for a year.

A year long gap in a CV will need to be explained. What you have here is probably fine, but it may just look better if you are able to say you were employed in the family business

Agree with others. It shouldn't be a huge deal, but you need to make it sound like you were doing very productive things and improving yourself. As you go along, this gets less and less acceptable. For instance, having a relatively unexplained 'gap' year or two post medical school adds a lot of difficulty to getting a residency spot.
