Rad onc also has a lot of people who don’t want to work solo to take care of 30 patients. That’s just the reality. People value their time and quality of life. I know some people here think that’s crazy but that’s just the reality. Many large practices are hiring more people and allowing more of them more days off

Completely agree. To me this is a solo doc practice and with efficiency (radformation helps) and good staff is not a crazy amount of volume.

With that said, I don't think it's a radiation-only phenomenon, especially in the employed physcian world. I think med administrators are being advised to hire more docs rather than have extremely busy docs. Part of it IMO is a power play, cannot consolidate too much volume/"power" with one doc.

Another VERY common misconception I've seen is when they try to apply surgical hiring principles to radiation - thinking if they bring in more rad oncs it will bring in more procedures. They think - well we hired another surgeon and they brought more cases or expanded our services and think that same thing works for rad onc....and it typically does not.

Rather pay 2 people 4-500k then 1 person 1 million for "optics".
Gotta love hospital admins.

I am sure they have no problem paying high dollar amounts for NSGY, Ortho, etc.

Academic departments intentionally overhire. My chairman years ago told me that adding extra docs "rustles up more birds from the bushes". If an extra 10 pts a year are over treated/over fractionated per doc, it is a great financial move, even more so at a proton center. last week, i talked an 88 yo with G7 (4+3) PSA 8 Decipher 43 prostate cancer into active surveillance. If I had 9 pts on beam at a proton center, he is probably getting treated?

I'm sure if any of us were magically turned into high level admins, we'd have no problem paying high dollar for these docs as well.

That's just good business...although for NSGY, a doc alone doesn't get it done, you have to have the staff to effectively run something like a Neuro ICU.

4-5 days week. probably best case scenario.

do you know any docs that are considered full time that are not on site less than 4 days a week?

im not counting boomer chairs/senior academics.

If you have a place like that, you need to always be advertising simply because it’s a hard spot to fill and the one that’s there could leave it any moment and you’d be f—-

These jobs are tricky. IMO you would need two docs that have like minded goals - to not be so busy and they value QoL over paycheck.

As long as that is the case it works. But if one person wants to be busier it can be a problem. And if one person leaves and the doc left behind is the one that doesn't want to be busy...well then there's a problem too.

I interviewed at a place like this about 10-15 years ago. Two nice, honest docs, one was retiring. The one staying didn't want to manage 30 but still wanted to work. They were honest with me and said it wasn't a good fit, you need to be busier (I was young and hungry). I think they were having challenges filling that job.

I think rural hospitals and practices should use this as a guideline: if your town has less population than this single apartment complex, the base salary should be $700k+

I would either treat or just observe. I wouldn't do the repeat labs and biopsies on an 88 year old. 88yo w/ 4+3 dz? If he looks like he's gonna make it to 100 I'm probably recommending treatment.

There’s spreadsheet on reddit that physicians put their salaries anonymously (the creator has claimed that they check if an entry seems fake). There’s a radonc salary from Frenso CA, not sure if it’s with this group though. The reported compensation is $675k

Wait why?

People move jobs all the time in rad onc to get to their more ideal location/situation.
 
This Fresno job has been posted in various places for years. Could be because its Fresno or something inherently bad about the position itself. I'd think anyplace in California paying $675K would have no trouble finding someone good.

Critical access hospital.

The only ones that were exempted from direct supervision requirements by CMS the entire time! Probably a great 3 day a week FT job for the right person...

They probably forgot to add 1:1 call and you are responsible for running your vacations by your RN Supervisor so that they can get a locums MD to cover you.
