I'm applying for IRB approval of a research project, and I'm supposed to submit a CV/resume to one of the various committees before I get to that stage. Nearly everyone reviewing my application is a physician or scientist, and nobody is going to be really impressed with what I've done. I also haven't done much in the way of "impressive" extracurriculars since college.

I'm basically listing it like this:

1. Contact info
2. Education: residency program, med school, undergrad (major, minor, summa cum laude)
3. Activities: recent volunteering (up to 2008), work on alma mater's admissions committee activities
4. Experience: research labs (both from college), clinical job that I held from college halfway through med school. Seems a little premature to list the research project in this section that I'm asking for approval on.
5. Honors: all I have here worth mentioning is some stuff like some good scholarships at the end of college.

Seems pointless to list references since they would just ask my program director, and I don't think they care about my hobbies or various skills. Am I forgetting anything that seems relevant? Anything I should not include?

It's really not that important, but I guess I'd like to update my CV on principle as well...

In this setting I would use the NIH Biosketch format. Go here and get the "Biographical Sketch Sample – Adobe Forms Version B" form. It's a sample one that you can just replace with your stuff. But it gives you a good idea of what IRBs and granting orgs are looking for in this.
