Urinary Problem

HI Everyone
I am not a urologist or even going to med school (I am starting vet school in the fall!), but my sister has been having a problem, and I didnt know if I could solicit any advice from a urologist.

She actually has been seeing a urologist in NYC for this problem, but I thought I would ask for some more advice.

In any event, she is 29 years old, and tells me she goes to the bathroom ALOT. She says she goes at least 3 times during the night- it wakes her up from her sleep. Additionally, she is having problems holding it in and can get a "drop or 2" in her underwear.

The urologist she is seeing is putting her on a VERY small dose of a certain anti depressant and says this should control her bladder.

Does anyone agree with this? Also- the urologist said that the "overactive bladder" term is a marketing tool used and that there is no such thing.

Any advice???? :confused:

Tell "your sister" to go to try an Ob/Gyn or a Neurologist. :smuggrin:
 
certain antidepressants (tricyclic antidepressants) have strong anti cholinergic activity, which is the same mechanism by which medications like ditropan, detrol, and sanctura work by.

Okay youre crazy! SHe does not need an ob/gyn or a neurologist, she is seeing a urologist. and second of all, it really is my sister.

You are so unsympathetic and uncompassionate :eek: its no wonder youre a med school reject.

Wow...wow....wow...what's with all this anger! :scared: I was kidding when I said "your sister". I did not really mean it. I am sorry if it really angered you.

As for your original question, your sister's urologist placed her on antidepressants for the reason mentioned by Applicant2002. But I feel that this alone is not enough since it is only symptomatic treatment. The question that should be answered is "what is the reason for her urinary incontinence? (ESPECIALLY AT THE YOUNG AGE OF 29)" For that I suggested an Ob/Gyn because, well, they are the "Female Medicine Specialists". Could the reason be an enlarged uterus compressing the badder? Could the reason be a weak pelvic floor? Could the reason be a neurogenic bladder? Who knows, but the primary problem need to be identified, otherwise you will only be treating symptoms.

But do not listen to me, I am just a lowly third year medical student. It never hurts to seek a second opinion from another specialist such as an OB/GYN, especially since they see cases like this day in and out.

Again, the urologists in this forum might have better advise.

Sorry I got so upset, I guess I just feel bad for her, she is only 29. Its not a neurological thing, I know that. I guess we'll see. Its not all the time, It is when she is stressed out.

I would suggest that your sister go and see a urologist with subspecialty training in female urology. This doesn't mean your sister needs to see a female urologist who is also a woman. She just needs someone who specializes in female urology whether they be male or female.

In order to do this, ask about the urologist training background. To truly specialize in female urology, one should do at least a one year fellowship in the field of female urology. Look for someone with such education.
 
I agree with the above post. I recommend seeing a Uro-Gynecologist (an Ob/Gyn who did a fellowship in Urogynecology).

Again, you'll note my bias here, but frankly, I would not recommend a uro-gynecologist. Urologists spend their entire time in training operating in the pelvis while gynecologists have spent 1/2 their training delivering babies.
 
Currently my sister is seeing one of the best urologists in NYC for this problem. I just was wondering if there were any other urologists out there with remedies other than the small dose of antidepressant.

Well, how is she doing? Is the tricyclic working well for her?
