Hi,
I was also finishing up my part 1 section of the application. I moved onto the recommendation section where I added 2 recommenders, but I could only download the LOR "Transmittal Forms," which does not have any questions like the Pharmcas or Western LOR forms do...

Also as for the transmittal forms there are like spaces for 3 separate recommenders... should I just cut that sheet I printed out into 3 sections??

BTW: I'm doing the LORs by snail mail, not online.

I'm stuck in the same situation...anyone familiar with this?
 
They didn't have this in the supplementals last year, so I'm not familiar with it. However, if you downloaded the forms I'd be willing to take a look at them if you'd like. Send me a PM and I can give you my email address. The other option would be to call USC directly. They seem to be nicer now that class of 2011 has almost started.
 
It appears it's much like the cover letter pharmcas requires for transcript requests. Basically you fill it out to ensure it gets sent to the Pharmacy Admissions office as opposed to any one of the other dozens of grad programs which use this same form letter. Basically fill it out with the 1985 Zonal Ave address and hand it to whoever is doing your LOR. They apparently are free to state whatever they'd like without any prompts in their LOR. Otherwise those instructions should have been included on the form as well. You can call USC to verify, but that's my take on the form based on the instructions at the top of the page.
