Yesterday a physician ordered Morphine 300mg po...because it was on a home med rec sheet...because the people that type them out are ******ed. Of course the real dose was 30mg CR. The nurse was sitting there on the phone demanding it be put in the Pyxis. And there I am...think "wtf?" to myself...

I have no doubts at all from my own career perspective. If pharmacists weren't in hospitals, people would die. Yet we don't get reimbursed in retail for doing the same thing as a tertiary entity....it is bull****....

Had a patient a couple weeks ago come in for an Rx of Novolog Insulin 3 mL TID. The person who wrote the script thought that 1 mL = 10 units.

A world without pharmacists would be a scary place indeed.
