Probably you won't need a J1 wavier after a fellowship as you don't get salary from the US government in the fellowship.

I am coming to the US for radiology fellowship and wanted to know how hard is to get a j1 waiver in radiology?

You can get a J1 waiver in radiology, but not as easy or as plentiful as in primary care or psychiatry.

You are right. Any ECFMG sponsored J1 will be subjected to the 2 years rule.
