VSAS TB Question

Hey all, I had a few questions on how I should fill out the TB section of the VSAS immunization form (not asking for medical advice).

My history:
I received the BCG vaccination as an infant and have subsequently tested + on PPD ever since, most recently in 2011 (10mm) which was immediately followed up with negative IGRA and CXR. Since then, I've only had IGRA or CXR with the most recent being last month (always negative). All of my providers since have recommended against prophylaxis (yes, I/they are aware of the CDC recommendations).

Anyway, my questions:

1) Which section am I supposed to fill out? Section A has the skin test section where you can mark +ve (even though it's supposed to be for negative skin tests) and provide IGRA results, but then Section B is the one designated for history of +ve PPD, doesn't let you provide IGRA results, and asks for prophylaxis.

2) Do schools require me to be on prophylaxis? Or will a letter from my doctor recommending against it be acceptable?

3) Not all programs have put their immunization standards up yet, but is it true that some schools require skin tests even though I have a negative Quantiferon and CXR? It seems silly to subject myself to a skin test when I know what is going to happen and then have to get ANOTHER CXR to show that I don't have active TB.

Again, I am NOT asking for medical advice. I'm asking how I should complete the form so my doctor can sign it, if I should ask him for INH or a letter, and if I should ask to get a skin test even though my recent IGRA and CXR were negative.

I know some of these might be institution dependent, but I'm hoping someone has had some relevant experience regarding this issue and can point me in the right direction. I just don't want to be delayed in processing my application or be rejected for something that I can take care of right now.

My advice is just have multiple forms of testing done, and document all it.

The AAMC form is just used for the convenience of both schools and students. It is not all encompassing, and many schools have different requirements.

My advise:
1) have a current ppd. I know it will be positive, but some hospitals require it prior to accepting other results. May be silly, but still true.

2) get another X-ray. It won’t hurt you and will save time later.

3) have a current IGRA blood test

4) put all of this on the AAMC form

5) do not get ppx abx for no reason

Its good you’re making sure this gets done now. VSAS immunizations can be a huuge pain, but you should be fine
