Question for anyone applying to U of Kentucky through VSAS:

Is there an immunization form that I am not seeing? Ive looked and looked for their form, but cant find it. Not sure what they want uploaded. Perhaps just all my immunization records? Any help would be appreciated.

Directly from their visiting students website:

•You must be current on all of your immunizations. including measles, mumps, Rubella, Varicella, Hepatitis and Tuberculosis.

I would just upload a signed copy of your immunization records from your student health clinic. If you didn't get the varicella vaccine, but had chicken pox, you'll probably need to get titers (some places will make you get titers for both varicella and hep B even with documented proof of vaccination).

ya, i saw that. i was just concerned there was a specific U of Kentucky immuno form that I was missing as other places I applied to have their own form that needs to be filled out.
