I was just curious...You know how many med schools have forms the evaluators can fill out and you sign on the forms to waive your right to access? Then the schools say evaluators can use their own letters in place of the forms...so what if the evaluator chooses to use his or her own? Do they typically just send their own without the incomplete form that you signed to waive your right? I just thought about this today and have sent out 5 secondaries. Does it really not matter?
 
At most schools they ask you to include their evaluation form with your waver on it with the letter. Just give it already filled out to the person writing your LOR and have them use it as a cover letter.

Thanks!! But, none of the schools I applied to listed that in the instructions. It just said that the evaluator may use his or her own in place of the form as long as it is written on letter head and is signed.--that is it.

Guess I will know if my evaluators didn't do that and it is a problem for the med schools.
