What reputation does your postbac program have upon medical school admissions? Do they keep track of past students/alumni and where they were accepted and subsequently went?
Your community service is lacking. The usual threshold is 150 hours for most medical schools and you would barely reach 100 (which could be the threshold for a handful of schools). Depending on what you are doing in the HIV clinic, some tasks could be non-clinical. But you need more in-person non-clinically adjacent activities such as food distribution, shelter work, job placement services, or housing rehabilitation.
