What are my chances?

First things first, you need to start shadowing physicians. Shadow MD's and DO's (if you can, DO's who perform OMM in their practice).

You're experiences are quite unique IMO. The hard part now is tying it together to create a singular theme that fits who you are and how you identify with medicine.

You're chances are pretty strong. Obviously the MCAT isn't doing you any favors but outside of that you're looking good. Would apply to basically all DO schools except WesternCOMP, Touro NY/CA, OSUCOM (unless from state), TCOM (unless from state), MSUCOM (unless from state), OUHCOM (unless from state), Rowan, CCOM/AZCOM. That still leaves you with a LOT of DO schools to apply to.

Don't hype up the global missions trip. It's two weeks long. Remember, you want to stand out. Every other applicant to med school has done some thing like this. I would much rather learn more about what got you into you tutoring business and how that relates to your desire to pursue medicine.

Your fine, alterscale gave you the best advice anyone can give. Just apply early and you have a chance at even the most established DO school.
 
I agree with AlteredScale also. Apply broadly to as many DO schools as your budget allots because you have great chance at DO schools. If the medcal mission trip is global medical brigades (something I have done) then it will be great experience. I learned a lot but to keep it in perspective, it was 1 week long. For me it was what I learned in preparation for it during the school year that also helped me understand foreign medical policies and care. And yes try to shadow physicians.

you're fine for most DO programs. maybe don't tell them about how you don't like to study or put work in though
