Not really sure how "ties" work, but I have lived in Georgia, Florida, and Maryland. I also have immediate family that lives in Kentucky, with both my parents going to school there as well.

You have enough IMO to avoid getting screened out, but my concern is you need to show consistent involvement with the community, not just racking your hours with the summer position. Push yourself to be more involved with H4H, especially with in-person opportunities. Try to volunteer at a shelter that can leverage your experience with Crisis Line work so that you can do it in-person; you don't need more on-phone or virtual experiences as much as you need to be face-to-face. I'll credit your Target customer service work a little to this end.

Hi, thanks for the feedback. I plan on doing H4H more consistently this upcoming year, and plan on doing around 200-250 more hours. I put this as “anticipated” hours, but my question is will this matter much to med schools?

Anticipated won't matter. Otherwise everyone would "anticipate" 1000+ more hours. Do what you can.

I see. I already submitted my primary to a “throw away” school, due to my later mcat test date. Is this something that should make me consider a gap year? I haven’t added more schools yet.
 
Since schools may have faculty who are more hard-core than forgiving, you can keep going. You never know as long as your activities support your purpose and mission fit. Your metrics have some value.
