What are my chances?

UTSW is a very selective school, with mean stats for acceptees of 3.9/34. Since your cGPA will be on the lower side, a correspondingly stronger MCAT score will help (though not to say that's essential, but it's a good goal). They are very strong on research, so having 2+ years should appeal to them, but be sure you begin taking on more and more responsibility so that eventually you might get your own project. Having a publication would be helpful. Typically MD/PhD is more competitive than MD-only.

Besides using your EMT certificate in a job or volunteer position, be sure that you also have experience in a clinical environment that a typical First Responder wouldn't have. And don't forget that physician shadowing is valued. Be sure to include a primary care doc.

You might eventually consider some off-campus nonmedical community service that you'd engage in on a weekly or twice monthly basis where you help those with poor economic resources through a cause you care about.

I don't think the lab switch would be a problem if you've completed a project in the first lab.

Your mom probably has contacts with pediatricians, who are primary care docs. Maybe she could help you set up with one of them, too, so you get a taste of office-based care.
 
If your leadership effort makes a major impact, then maybe. It isn't more shadowing and clinical experience they'd want to see, but rather, substantive research, amazing leadership, and above-and-beyond nonmedical community service. And maybe a standout hobby, sport, or artistic endeavor.
