Hello, I am currently an undergraduate student at the University at Buffalo (UB). I transferred there after studying at University of Toronto (U of T) for two years. I recently received my green card and I will soon become an instate resident of NY.

At U of T for 2 years
3.59 GPA- Not a full course load (around 9 course credits/semester)
Unfortunately, I took organic and inorganic chemistry in the summer
Volunteered at a hospital for the summer (110 hours)
Math tutor at a high school (25 hours)
Traveled to Costa Rica and Nicaragua to provide medical relief to rural areas for a month.

At UB
for 2.5 years
3.93 GPA- full course load (15 course credits/semester)
Volunteering at multiple hospitals- even overnight (more than 250 hours)
Volunteering in areas other than medicine (less than 40 hours)
Shadowing doctors- many specialties (more than 90 hours)
Research for 1 or 1 year + summer
Coordinator of a club that takes UB students to Guatemala to provide medical relief (same organization as in U of T)
Officer of The table tennis club
Active member of three honor societies- one of which is pre-health

I have NO paid work experience and NO internships

Overall- projected
3.81 GPA
30 MCAT balanced
Average reference letters

Despite my shortcomings while I was at U of T, will there be a realistic chance that I will make it into an MD program school? More importantly, is there anything I can do to improve my chances?

Wait to apply until after you establish NY state residency for the additional statistical chance of success that will give you at NY state schools.

Looking at past AAMC data, one can extrapolate that an applicant with a 3.8/30 has a 58% chance of an MD acceptance.
 
Is there anything else I can do to improve my chances- other than applying when I get my instate residency?
