What are my chances?

Okay, here it goes.
I am an Asian American currently attending a big ten school.

My sGPA and cGPA are both 3.8. But I did get a C+ on Physics II... 🙁
Just scored 39Q on MCAT! yay!
Extensive research experience (2.5 yr) and will be published in an undergraduate journal and have attended symposiums/ conferences, etc.
General volunteering through Student Council and various clubs.
Volunteered at local hospital for a year.
Worked 5+ hours per week through sophomore and junior year.

How are my chances?
My top choices are University of Chicago - Pritzker and Northwestern - Feinberg. But I'm not sure how my C+ in physics will appear to them... Would the C+ affect my chances even though I have a decent GPA? Should I aim for lower level schools? What other schools do you suggest? I also feel like I don't have much clinical experience as other applicants I have talked to...
 
Great GPA and MCAT, congrats. I'd say that you have two concerns:

1) Clinical experience appears slightly under average. Not too hard to take care of this, though.
2) From what you've posted, you come off as VERY cookie cutter. I don't see anything in your app that distinguishes you from every other typical applicant (except your high MCAT, of course)

That being said, I don't see any possible way that you wouldn't be accepted somewhere...the top 20 is a crapshoot though and you need to find some way to make distinguish yourself.

If you haven't done any physician shadowing, it would be well worth your while to get this in quickly before you submit your application.
Any leadership or teaching?

I doubt the Physics C+ will be an issue since your overall GPA is good and your PS score proves you "got it."

I am trilingual (fluent in Korean, English, and French) and I am also the founder and the president of Korean language club that gives free Korean lessons for anybody who is interested in hopes to bridge culture gaps between the diverse students at my university.
I also give regular concerts at a nursing home with some of my friends since I've been playing various instruments from young age.

Could these be a "wow factor" for the admissions committees?

You have a great GPA, and high MCAT. To me I think you seem like any other med student applicant. I think your chances are above average. You have great experience so far with teaching. Get that physician shadowing in and get a letter of recommendation from the physician. Add it to your application and go apply to Hopkins or Stanford. ( You have the potential. ) I doubt you would have any issues getting into Pritzker.
