Research matters for top tier schools but shadowing, clinical hours and non clinical hours are more important for most schools.

I have 400 hours volunteering at my hospital but its mostly nonclinical, so I need to grind out clinical thank you!!!

Since you seem to be interested in Stony Brook, I'll toss this out there as I got accepted there.

You should have some research. I had around 750 hours and 2 posters, no first author.
For non clinical, make sure you have some underserved volunteering, like food bank and working with Veterans. Stony Brook has the Long Island State Veterans Home so they look favorably on working with veterans. I had food bank, food distribution and volunteering with veterans. 250 hours is minimal. I had around 400-500.

You should have some leadership (TA, Tutoring, etc.)
For clinical I'd recommend 350+. Not sure if they have a specific cutoff, but as a minimum I'd recommend 350 but preferably shoot for 500+.

Shadowing is not super important. Most folks will only have around 60-80 hours. It will generally overlap with any MA,CNA or scribing work so don't worry about getting too many hours of pure shadowing.

I don't know if Casper mattered, but I did have Q4. It also helps to be above their 50th percentile for GPA and MCAT. They did interview 589 out of 2012 in-state applicants and 75% of their accepted students are in-state.
