Sometimes I see standard condition as 25 degree Celsius and sometimes it's 0 degrees Celsium. What are the different conditions I should be aware of when I do a passage? A lot of times, I get the temperature wrong because I'm not completely sure what the condition simply means.

If it's a temperature related problem they usually give you the temperature (be sure to convert it to Kelvin depending on what formula you are using - i.e., PV = nRT). If temperature is not given, it's usually 25 degrees C (about room temperature). If you see STP, you should use 0 degrees C.
 
Pretty sure the only standard state conditions are 1 bar and 1 M. Temperature is specified, but usually its 25 C, but is not a standard state condition.
