For most schools, the exam taken is an NBME Subject Exam at the end of that MS-3 clerkship. It's a standardized exam, unlike a professor-written exam, written by the National Board of Medical Examiners. The rotations that have NBME exams are: Clinical Neurology, Family Medicine, Medicine, Obstetrics and Gynecology, Pediatrics, Psychiatry, and Surgery.

Your score on the NBME subject exam usually contributes a certain percentage to your grade overall in the course. At many schools, in order to qualify for "Honors", you have to get a certain exam score.

Since when does Snape give gryffindor points? I remember him trying to take points from them when they had none

I googled "10 points for Gryffindor" at work...panicked at the search results and clicked the first SFW pic I saw.

(Hint: the search results involve a lot of pictures of Hermione/Emma Watson)

You learn a ton. I feel like what I learned M3/M4 dwarfed what I learned as an M1/M2, but much of it is less "testable".

Third and fourth year is when you learn medicine.
