What does mean a PSA increase?

Hello, my pathology professor gave us a clinical case. In this case a 63 years old man shows up with dysuria, nocturia and urinary obstructions to repetition, PSA is 1,7 ng/ml but after a year it increases to 3,9

What does this increase means?

This level of PSA increase may mean nothing or it may mean a lot (like slowly brewing Gleason pattern 9 or worse). The man needs a biopsy, 6 quadrant or better yet 10 quadrants with dual HMWK CK and p63: 88305 x 10; 88344 x 10. If all you have is a hammer, everything looks like a nail.
