What error rate is acceptable to you?

I just discovered the first error of my professional career today. I have been practicing for about 2 years now. Messed up directions on a script for Bumex (dispensed 1 QD instead of 1 QOD). Caught the error on post fill audit too late to reach the patient at night, so planning on calling the doc/patient tomorrow.

So what's acceptable to you? I feel sick to my stomach right now but I know I wasn't going to go 30 years without making an error. Hoping I can reach the patient quick.
 
For me I wouldn't say "acceptable" is the proper term. No error is acceptable for me, its something that always bugs me and I will always lose some sleep over it. With that being said, we all make mistakes and we only catch a small % of the mistakes we make.

If that is the first mistake you've caught in 2 years, I would say you're doing pretty good.
 
No error rate is acceptable to me. I check everything.
 
12%, but I start to tighten up my checking process after 10%, because I like to leave myself a cushion.
 
There is no "acceptable" error rate. You will make errors, and the literature suggests that it the overall rate is around 1%, of various magnitudes. All of us have made errors, you're lucky enough to have noticed it.
