.....what? How on earth are you proposing to use pt outcomes or M&M or any other clinical criteria to judge medical students applying to residency?

No I mean in residency there are measures for a "good clinician," you said most people would disagree. My point is there are objective measures once you are a physician or resident.

Hell I'd think a M3 retrospective random note review would be more predictive than step 1, but who knows.

Oh I agree with those measures once you're a resident...but the discussion here is what do we use instead of STEP, not measures of a good clinician later on.

The note review runs into the same problems as 3rd year grades...a lot of variation in who you get writing the note, how its worded, policies within each school, etc. Sorting through that would be impossible for PDs.

For my money, STEP 1 may not give you the best clinicians....but it gives you a reasonable measure of who gives you a mix of intelligence and diligence. And that is probably all you can reasonably expect out of a medical student that's going to develop through your residency program.
