Write the school a letter of interest explaining why you decided not to matriculate before and why they KNOW you'll actually attend if accepted now. Say that you wanted both an MPH and an MD in order to accomplish your future career goals, and getting them in that order made more sense financially.

The letter should focus on why you want to attend their particular school. (e.g. What programs of theirs interest you? How would your experiences allow you to contribute to these programs in a meaningful way?) Bonus points if you somehow connect your MPH to one of their programs.
