What is the most common cause of painless hematuria in adults???

I have seen two answers in U World and Kaplan Qbank

1) bladder mass
2) BPH


which is is correct???

3. IgA nephropathy

but if you are saying an adult is 40 or over then i would go with bladder mass
 
bladder mass....bph doesn't often give hematuria, just hesitancy in urination
 
"Painless hematuria" is very buzzy for bladder mass, especially with older age... if you see any mention of smoking in the history this is a dead-ringer as well.

I came upon that same Uworld question yesterday night, but the site was down,

The Uworld question said that the most Common cause of microscopic hematuria in older men is BPH.

So, the take home is painless macroscopic hematuria: bladder mass
painless microscopic hematuria in older men: most common cause is BPH.
