The undergrad GPA is the only blemish on your application. You will be fine, however. Many programs do not offer PhDs in epi or biostats, so your choice of programs will be limited.

How do you feel about a post-bacc or smp to make up for my low sGPA? Do you think that would be overkill?

This is a wasted effort, particularly when your application is strong enough that you should be a good candidate for many programs around the country.

The key is you have a grad GPA of 3.9 in the area of your interest. I am making the assumption that you have taken a number of rigorous grad courses, and the 3.0 BCMP GPA does not include C's in things like Calculus & other math courses. The 39Q MCAT will be catnip for admissions committees, and will go a long way towards mitigating GPA concerns.
