Currently, I am PGY-1 categorical surgery resident. I'd like to transfer into a CA-1 position starting in 2010, but I don't know how realistic an option that is. Is it better to just apply through ERAS now for an advanced position, CA1, and do research for year until I start in 2011? Is this a common scenario? Is there a website that shows open anesthesia positions like there is for surgery? Any ideas or advice would be appreciated.

Will I be at a significant disadvantage applying only for advanced (Ca-1) positions, since this would technically give me a year of sitting on my hands? I imagine this could be a problem for some programs; however, I was thinking about suggesting research involvement for that year in the interested program. Is this just a foolish idea, and should I really consider apply to categorical positions and redoing a transitional/prelim year? Again, I am currently a PGY-1 categorical general surg resident.
 
Go ahead and apply thru ERAS. But know that many programs have CA1 spots open next year for people just like you who are switching out or didn't really know what they wanted to do and did a TY or prelim year. What you do when you interview is you tell them that you are interested in an out of the match CA1 position for next year if they have one. If they want you and have spots they will offer you a spot so that you don't necessarily have to sit out for a year. Now my med school UTMB used to do that before hurricane IKE. Don't know if they still do that. But many hospitals do have this option. Good luck and great that you came to your senses. :idea:

Why not just try to get a position in CA-1 without going through the match? I don't know where you want to end up geographically, but for example, Univ of Wisconsin offers 3-5 outside the match positions for people interested in transferring. If I were you, I'd try to skip the whole match process.

Is there a place where you find programs with extra slots available for people who want to transfer? Or do you just have to email programs directly and ask, expressing your interest...

Also, anyone know if these positions are typically offered in more major cities (NYC, chicago, san fran, etc)?

I think it depends. I had previously seen open spots in Anesthesia through resident swap. I also looked at Wisconsin's website and they said they offer spots outside the match. But otherwise I guess it's calling and finding out.

Going through ERAS would mean I would lose a year with advanced programs (I'm currently doing a categorical which would count as my intern year). Is it realistic that I could find a spot by contacting advanced programs for a CA-1 position starting 2010? I'm also geographically limited (NYC) so this seems like a risky approach...

I'm just wondering if its common for other programs to have available spots like the U. of Wisconsin.

I haven't gone through this, but why don't you just call all the local programs (or email) and ask them of they accept people switching from other specialties outside the match? You might then have an idea of what your options are locally.
