Good golly, some surgical tech wasn't doing their job. Not to mention the surgeon's neglect. This should never happen. A surgical tech counts all instruments and sponges and is suppose to keep an acruate count. People were clearly sleeping on the job. That poor patient.

Why didn't the tech count all his or her instruments? And why didn't the surgeon ask if the counts were correct? Every surgeon I have been with in the OR asks if the counts are correct before closing up the patient.
