when giving the LOR forms to attending,

along with cover sheets, blue pens, thank you letter,

should i include blank envelopes too?

they're not required to use special hospital-tagged envelopes right?
white envelopes and regular stamps will do?

Should be fine, although you could probably get a hospital envelope from the office when dropping it off and put your own stamp on it.
