.A 19-year-old female was admitted to the coronary care unit after an aborted sudden cardiac death. The patient was awakened with what appeared to be a bad dream and suddenly collapsed. The fall was witnessed and her mother started CPR immediately after calling 911. Paramedics arrived rather quickly and found her to be in VF. She was successfully defibrillated with one shock at 200 joules (biphasic). She remained comatose and was intubated and transported to the hospital..
.She is currently on the ventilator but withdraws to painful stimuli. Her pupils were dilated, but reactive to light symmetrically, her corneal reflex is present on weaning the sedation. Her past medical history is remarkable for 3 brief fainting episodes. She is not on any anti-depressent or oral pills or any other prescription medication. The mother denied knowledge of substance abuse. Her mother remembers one of the maternal uncles having had a ICD put in after he suddenly collapsed at work. .
.What is the most likely diagnosis at this time? .
