When should I get doctor's recommendation?

I will be applying for 2011 dental schools.. so I will be filling out apps and all the necessary paperworks during 2010 June.

I am actually working as a dental assistant at a dental office and would like to know when I should get the recommendation from the dentist.
Should I get it end of this summer even though I am going to apply to dental schools 2010 June? or should I wait until next year and ask him to write the recommendation right before the apps?

If I should get the recommendation at the end of this summer, where am I suppose to get the recommendation?
Where..? I mean is there a certain form for recommendation or.. should I just give him a piece of paper to write me a recommendation?

Looks like you have a while so there really is no rush. You should ask him for it when you feel that he has all the information he needs about you. If you just started, you should wait a little until he really gets to know you so that the letter can be that much better. Whenever you feel your relationship with him is best, you should ask for the letter.

My school kept a file for all the letters that we needed. So instead of us shipping each letter out individually, they shipped out the letters together to the dental schools instead of 1. They basically just organize everything for us. There was a specific form for my school. There's lines provided on there for the person writing it and all you needed to include was your name and your writer's information and signature. You should check with your school's careers center to see if they have the forms.

Looks like your dentist LoR will be easy. You should start finding the science professors because those will be harder. Those profs are always busy so make sure you ask them ahead of time.

I'm kind of in the same boat: I'm shadowing a dentist now once a week, and I'm unsure of when I should ask him for the LOR. I'm just shadowing, so I'm not really sure how long I should stay. I kind of get the feeling that I'm in the way, and I don't want to over-stay my welcome. Would it be too soon to ask him for a letter this summer, even though I'm applying next June?
