I have matched and have read that it is best to notify the program as soon as possible that I am pregnant. Can I tell them now or should I wait to see and sign the contract?

Great.

Technically if you matched, they cannot refuse to send you a contract because of your pregnancy.

However, my answer really depends on when you are due. If you are just barely pregnant, then waiting a month or two to get your contract probably won't make a big deal of a difference. But if you are delivering in July, you should let them know ASAP. One could imagine them trying to shaft you adn refusing to send you a contract.

I'd call and see when they are sending contracts out...if its soon, then wait. Otherwise, let them know sooner rather than later.

It's a good idea to let the program know sooner rather than later as scheduling changes can be a nightmare. But if the approximate due date is known then some creative scheduling can be done ahead of time so that if something happens, changes will be relatively painless. Or if nothing bad happens and the blessed event occurs on time everything will be in place and no changes will be needed.

Programs try to start setting up schedules in April or May so that they are ready by the end of May early June. You will get a packet of forms to fill out including a schedule request, which should have an area for special requests. Be sure to include your due date and request elective time around then (during the month or so prior to and your due month).

Your PD and program Chief Resident are your allies (despite what others in the forum may say). Keep them informed.
