Many questions about interviewing.

If you have two or more specialties you are applying to, and you have only one interview in one of the specialties (example one Internal Medicine interview all others Family Medicine) should you tell the Internal Medicine interviewer this if interviewer asks?

What can be said instead?

Do you mention the other specialty ever, even if you have 5 Internal Medicine and 5 Family Medicine?

Do you talk about other programs ever if you are asked?

I'm puzzled why you would do FM and IM... both matches are such that if you have a decent record, then you can get into a good program!

I would NOT mention two specialties... it shows you are not committed to either one... an application divided cannot stand logically.

For other programs, I would give a generic statement saying that "I am interviewing at a broad range of programs... what specifics would you like to know?"

If they ask about a program, ALWAYS be positive about other programs. Everyone knows you are interviewing at multiple places... no sense in being evasive and negative about it! I would redirect to the program you are interviewing about and say how you are excited about interviewing here because the opportunities (which you should know). Don't compare programs to programs... it's like apples and oranges... and your interviewer may have went to residency or something like that at that program... so just avoid digging yourself into holes!
