where do I find LOE matching form?

I heard you need to print out LOE matching form with evaluator's name and specific barcode. But, I can't find this form online. Where do I find this form? And is attaching this form mandatory?

The form is mandatory. You have to wait for aadsas to open for the 2008 cycle- which is in may. after you enter some information about yourself and ur school, you will be directed to the section of the application that allows you to print out those forms
