Which GPA to put down?

Sorry to bother everyone once again but this is something I have been debating so I thought I would get some extra opinions on the matter. Officially, I have attended three universities. I attended one during high school (high school classes that carried college credits), one for my freshman year, and then I transferred to my current school for sophomore through senior year. I am of course sending all these transcripts but I was wondering what I should put down in the self-report undergraduate GPA boxes? Should I do my GPA at my last school, do the math and figure out what it would be including the second school, or even include the credits from high school (which are unfortunately all B's because that is all I needed for a 4.0 weighted grade).

Def don't bother including the high school grades.

I don't think you need to include college course while in high school in your grades (I didn't) but you should submit those transcripts (I did).
