Which programs require supplemental info?

Hello

I know that some of the programs listed on SF match require supplemental info besides the CAS application- but I figured that most of the programs will contact you to ask for this info (pictures, $$, CV, etc.) after receiving your initial application.

Do you know of any programs that want this information from the start? I've been looking through the residency websites of the programs to which I've applied to try and find out. Haven't found anything as of yet, but I thought I'd ask around just in case anyone else knows-

That Emory link wasn't working for me above...

but I received a "we received your application" letter from them in the mail (first mailed one I've gotten). They want a picture (no size stated) with your name and SFMatch number on the back.

hey i called them cause i'm away and i don't get regular mail. they want something like passport sized. they said wallet size is fine too. just nothing bigger. no 8x10 glossy prints of you and your glamour shots.

i also email cole eye institute and they said no they don't require additional info apparently its just a rumor.....
