medicine and surgery are the "most important" for some specialties (i know this is the case for derm) but it's not like the others don't matter. maybe med and surg are looked at first, but 2 A's and 4-5 B's or B- is still nothing close to all A's. It's probably just that if you get a low grade, it shouldn't be in medicine or surgery. again, this varies by specialty and i can't tell you exactly what specialties put emphasis on med/surg. just my impression of how this works...
