A list is fine, but the lack of a hold or R at schools that have issued them very likely means you have not yet been reviewed rather than you've survived any cuts, given how many applications have been submitted and how early it is in the cycle. What reason would they have for reviewing you and then NOT giving you a hold, R or II if that's what they are doing for others?

I honestly don't know how many use it. The only way to know for sure is to see if it is reported on the school specific thread. What I do know is that if you have applied to a school that does send them, and Rs, then not hearing means they haven't gotten to you yet, rather than there being a special category just for you that's a secret!
It's where a school tells you your application has been reviewed and is on hold, meaning no II and no R. It's way more transparent than just reviewing your app and putting it aside without telling you anything if you aren't receiving an II. Holds later either become IIs or Rs after more apps have been reviewed. The same thing can happen after an interview if they aren't ready to make a decision one way or the other, or to just put you on the WL.
Actually, they mean the same thing. It's a notification that the application is in limbo!
Remember, many schools just place an app in limbo with no communication at all. It represents increased transparency, but means exactly the same thing -- no action on the application. It does, however, tell the applicant that the application was looked at versus still being in a queue.
One school that gives out hold is UCSD. As they review apps they put them in II, R and H buckets. They revisit the holds in January or so and send some IIs from that list. Check UCSD threads for previous years.
