Which states have the most Physician-only cases?

As the title states. What states have the most physician-only cases, rather than medical direction/supervision?

I know California is #1, but what other states have opportunities for doing own-cases? Thank you.

The state of denial. Hands down the most physician-only cases. Followed closely by Fantasyland, any parallel universe, and the Land of Make Believe.

From the top of my head, CA, WA, OR, AZ, CO, HI, NV, IN all have big physician only groups. Other states might have small pockets or single practices.
