I think the worst ones are the ones who are cocky and think they are better than everyone else...

I voted other to signify none of the above.

I can honestly say that I don't really look at one field being worse than another in terms of competence. I think each field is usually very good at what they do. I may have an opinion of worse as it pertains to my likes and dislikes in pts and lifestyle. But we all went to the same medical schools and learned the same stuff before we specialized. So in the end I bet we all do our respective jobs pretty well.

Now, lookee here...we all got to be honest with ourselves and acknowledge that the worst docs in the hospital - in terms of providing patient care - are the orthopods. Even the orthopods themselves will admit this. They are all these great, funny, super people who are fantastic at what they do, but they forget everything in medicine that didn't involve screws and flouroscopy by the time they hit the end of intern year.

Today I dropped in on a previous patient of mine who was in following an ortho procedure. Although he'd been eating for days, his IV (normal saline) was still running at 150cc an hour and he was making almost two liters of urine a shift. Thank god his kidneys were good; otherwise I suppose he'd have just spiraled into pulmonary edema. His potassium was 2.3 and magnesium was 0.9. My first clue that his electrolytes were probably askew was the myotonic jerking and frequent PVC's he kept throwing. His blood pressure was running in the 170/100 range, yet he'd not been started on any of his home antihypertensives. Of course, none of this mattered to him, since the 2mg of ativan he was receiving every shift left him pretty much obtunded. Being that he's in his 70's, it ought to take him a good 5 to 7 days to clear those active metabolites.

On the other hand, his knee looked great 🙂 .

I'm sorry, but I think I'd be pretty weird too if all my patients were either dead, or I only saw small pieces of them day in and day out. 😉

That is a great ortho story...classic. I had considered ortho but then had an experience with an orthopod attending on an elective who when confronted with a patient who was allergic to Lovenox decided to call the hospitalist to manage this problem. His reasoning was that he didn't know how to manage a heparin drip and wasn't used to monitoring the PTT for anticoagulation. At that moment the light shone for me and I knew that I could not do ortho.

This is not to say that orthopods are not very talented doctors in their own way and also not to say that there aren't pods out there who know how to manage medical problems. Just isolated moment for me that kind of makes a decision for you. 😉

when a roomful of ob/gyn residents couldn't give the #1 step to treat DKA, it made me wonder...
 
Something is wrong with a poll that shows more people think that surgeons are worse docs than the PM&R people. Good grief.

Oh an as too ortho, the most plausible floated theory that I have heard is that there is a ritualistic pithing ceremony when an orthopod finishes their intern year. It would explain so much.

This poll is biased. Of course IM docs won't vote IM and they are more than all the other docs. Same with FM docs.

I think that Rad docs are the worst of all. I have worked with a Rad doc through my research and he is so full of himself.
 
Rads are by far the worst on the list. I didn't vote for them because I don't consider them docs.
 
Like any other specialty, Rads guys know their ****. Hey, if I could stand reading films all day and getting PAID , I would do it. Too bad it bored the hell out of me. Oh well, I guess someone has to do it. I prefer to read my own films (ortho) anyway.
 
I voted for "Other". There should be an option for "The ones that don't care."
 
sorry i have to comment. as i read all the above entries, i realize that everyone seems to be defining a "good" doctor as someone who can manage medical problems. but that's why people specialize and go into different fields. orthopods are good orthopods. ob/gyn docs may not know how to manage dka but they know what to do when a 30 week pregnant woman has late decels. i could go on and on. the worst docs are the ones who are not knowledgable in their speciality or don't care about the patients.
