usually ones that don't 'make it off the island' is due to failing STEP 1/medical school courses. The Caribb curriculum is as difficult as American MD, which is why they have high fail rates. Students with lower stats naturally are more likely to be unable to handle the medical school curriculum.

At least this is the case for the big 3 (big 4?)

The reason one of my friends gave was that she couldn't wait anymore. After getting 2 masters degrees, I think she just wanted to move on with her life and her career. I can see where she's coming from, but I still disagreed with the decision.
 
Putting in an "extra couple of years" doesn't mean you'll get an acceptance.
